NEW REGISTRATION PROCESS STARTING OCTOBER 1, 2022

AS OF OCTOBER 1, 2022, CMG is updating its registration process and you will receive a text/email to confirm your appointment and fill out registration questions.

THIS MUST BE DONE PRIOR TO BEING SEEN IN THE OFFICE FOR YOUR APPOINTMENT

FOR WELL VISIT APPOINTMENTS, you will also need to FILL OUT YOUR FORMS VIA THE PATIENT PORTAL BEFORE THE APPOINTMENT.
