What is a Patient Portal?
This document explains what a patient portal is and how you can access it.
A patient portal is an extension of the electronic medical records system and usually hosts important information about patient health records. You will need to set up your patient portal access before you can log into the Healow App, and you will use the same credentials to access both the patient portal and the Healow App. Below is a list of items you can easily access on the patient portal:
- Dashboard
- Access some of your most recent health records directly from the dashboard.
- Personal Health Record (PHR)
- Download, share/email, and print your records.
- Visit Summary
- Download, share/email, and print your visit summaries.
- Current Medications
- Request medication refills.
- Referrals
- Find referral summaries ordered by your CMG provider.
- Lab/Diagnostic Results
- Access lab/diagnostic results ordered by your CMG provider.
- Immunization History
- Growth Chart
- Documents
- Download documents submitted to you, such as 3231 and 3300 forms, etc. We submit documents to the portal for patients to view and download a PDF version.
- Upcoming and Historical Appointments
- Questionnaire for Upcoming Visits
- Complete required forms prior to well-check visits.
- View and Print Your Statements
- View and print statements. Payments cannot be made through the patient portal but can be made via text when we contact you about an outstanding balance or on our website (https://www.cmg-pc.com/).
- Reset Password and Other Account-Related Information
Patient Portal Access:
You can access the patient portal via two methods:
1. Desktop Internet Browser
To access the patient portal, visit our website at https://www.cmg-pc.com/ and click on "Patient Portal" in the top right-hand corner.
- Your login credentials are generated and provided on your first visit to the practice.
- If you lose your access credentials, please contact our practice to reset the password.
- The most convenient and alternative way to access the patient portal is using your mobile phone. See the following screenshot.
- Select "Using Mobile Phone," enter your patient’s information, and click "Submit." On the next screen, select your phone number to receive a one-time passcode to access the patient portal.
2. Healow App
- To download the app, visit the following link https://healow.com/app.html and use the following practice code to access CMG’s patient portal environment.
- Practice Code: JAAEBA
- You will need login credentials to access the Healow App, which are provided at your first appointment.
- If you lose your access credentials, please contact our practice to reset the password.
- The Healow App will ask you to create a PIN, which is required to access the app.
